Re: ISAKOS Approved Teaching Center

Dear moises Cohen:

It is my sincere pleasure to congratulate you on the acceptance of the Instituto Cohen de Ortopedia, Reabilitação e Medicina do Esporte as an ISAKOS Approved Teaching Center. The application was approved and the teaching center will continue to be referred through 2011. If you wish to uphold your status as an ISAKOS Approved Teaching Center, you must reapply in December 2011.

As an ISAKOS Approved Teaching Center, you will be listed with other approved centers on the ISAKOS Web Page www.isakos.com and may be spotlighted in the ISAKOS Newsletter.

Please find enclosed the Teaching Center Certificate and Evaluation form. Please ensure your attending Doctors complete the form at the end of each course that you direct. As a Teaching Center, you will need to return the completed forms to the ISAKOS office.

In an effort to ensure that visitors to the ISAKOS website receive the most updated information, we are asking that all Teaching Centers have a complete online profile. Please e-mail the following information to the ISAKOS Office (isakos@isakos.com) to include on the ISAKOS website:

1. A drawing of your country that illustrates where your teaching center/city is located, for possible use in a future newsletter (preferred format: jpg or tif)
2. High Resolution Photographs of your teaching center, for possible use in a future newsletter (preferred format: jpg or tif or eps)

Again, congratulations on your acceptance as an ISAKOS Approved Teaching Center. We hope to see you at the 8th Biennial Congress in Rio de Janeiro, Brazil on May 15-19, 2011 and look forward to your input and sharing the opportunities ISAKOS provides.
